For a copy of a Tennessee birth certificate, please print and complete the appropriate form, enclose a copy of a valid government issued photo ID, and send a money order or check (payable to Tennessee Vital Records) to the name and address below.
The Vital Records Office keeps birth records for 100 years after that time, the records are sent to the Tennessee State Library and Archives for public access and family research.
