| For a copy of a Tennessee birth certificate, please print and complete the appropriate form, enclose a copy of a valid government issued photo ID, and send a money order or check (payable to Tennessee Vital Records) to the name and address below. |

| The Vital Records Office keeps birth records for 100 years after that time, the records are sent to the Tennessee State Library and Archives for public access and family research. |
