By Texas law, birth certificates have restricted access to family members and their legal representatives for 100 years from the birth.
For a copy of a Texas birth certificate, please print and complete the appropriate form, enclose a copy of a valid government issued photo ID, and send a money order or check (payable to DSHS) to the name and address below.
