By Arizona law, death certificates have restricted access to only family members or their legal representatives. When ordering a death certificate, you must prove the relationship or legal interest to the decedent and submit a copy of your government issued picture ID.
For Arizona Death Certificates, please print and complete the appropriate form and send a money order or check (request will be delayed until check clears) to the name and address below. Please submit copy of picture ID and proof of relationship, as stated above, with your request.
