| For California Death Certificates, please print and complete the appropriate form and send a money order or check (payable to the Office of Vital Records) to the address below. All requests must be submitted by mail or fax. |

| Due to budget cutbacks and staff shortages, processing time for copies of vital records is 15-20 weeks. The fee covers a 10-year period search. |
