For California Death Certificates, please print and complete the appropriate form and send a money order or check (payable to the Office of Vital Records) to the address below. All requests must be submitted by mail or fax.
Due to budget cutbacks and staff shortages, processing time for copies of vital records is 15-20 weeks. The fee covers a 10-year period search.
