The Indiana State Department of Health (ISDH) issues certified copies of death certificates beginning with 1900. Prior to 1900, records of death are filed only with the local health department in the county where the death actually occurred. For an Indiana Death Certificate, please print and complete the appropriate form, and send a money order or check plus to the following name and address: |

| The fee is a non-refundable search fee, and includes (1) certified copy if found. There are no indexes to the state death certificates form 1900 through 1918. The specific city or county of death must be provided for searches in this period and the search is limited to one county for each $4.00 search fee paid. |
