Requests for New York State Death Certificates can be divided into two types: Deaths within one of the 5 boroughs of New York City and those for the rest of New York state: Please note the 5 boroughs are Manhattan, Queens, Brooklyn (aka Kings), Staten Island (aka Richmond), and the Bronx.
For a New York City Death Certificate (1949 - present), please print and complete the appropriate form, enclose a copy of a valid government issued photo ID, and send a money order or check (NYC Department of Health and Mental Hygiene) to the name and address below. |
